Medical Certificate (MEDIF)

Essential information for those who need special assistance

For passengers who require special assistance, whether temporary or permanent, we offer all the necessary support for your trip. In these cases, it is necessary to fill out the Medical Information Form (MEDIF), detailing all the information provided by the doctor.

Attention: the old version of the MEDIF form (5-page template) will be accepted until August 10, 2026.

MEDIF medical certificate
For passengers who require medical attention or special equipment during the flight, it is necessary to complete the MEDIF form, which must be completed by the passenger's doctor and sent to GOL between 15 days and 72 hours before boarding. After careful analysis of the form, our team issues an opinion that certifies the passenger's suitability or not to travel by air. If there are any doubts regarding the completion of the MEDIF, or the need for additional exams, they may be requested free of charge from GOL.

Check when it is necessary to complete the MEDIF form
  • Recent surgeries;
  • When the passenger develops any unusual behavior or physical condition, which may have an adverse effect on the well-being and comfort of other passengers or crew;
  • It may be considered a potential risk to the flight's safety or to your own (examples: rerouting and extra landing);
  • Need medical attention or special equipment to maintain your health during the flight (oxygen or stretcher);
  • You may have your medical condition worsened during or because of the flight;
  • You need medical monitoring or medical treatment on board.

Present a picture of neurodegenerative diseases and cognitive difficulties.
